Monitoring forms

A monitoring form is the structure behind everything the apps collect. It decides which questions a monitor answers about a school, its head teacher, each class teacher, the lesson observation, and each pupil assessment. Setting these up is administrator work, so most of the buttons on this page appear only when you are signed in as an administrator. Everyone else sees the same pages largely read-only (the Print and Add to all country projects actions remain available).

The form hierarchy

Forms nest inside one another, from the school at the top down to a single assessment:

• A School Form Specification is the top-level form. It holds the questions asked about the school itself and its head teacher, and it contains the teacher forms.
• A Teacher Form Specification holds the questions asked about a class teacher and the lesson observation, and it contains the pupil forms.
• A Pupil Form Specification is the form a monitor uses for one pupil. It points at an Assessment Definition, which is the actual reading or numeracy test, split into assessment sections (Sounds, Words and so on), each with its own script, scoring guidance and questions. Questions themselves are not created inside a form. They live in Question Banks and are added to a form by reference. See Question banks and Monitoring questions.

Where assessments come from

Pupil forms reuse a shared Assessment Definition, so the same reading test can sit under many different pupil forms. Assessment definitions are managed on their own page, covered in Assessment definitions.

The School Form Specification view

Open a form from a country or project (country officials also get a Monitoring Forms group in their sidebar listing the country’s forms). The page header shows the form’s name with the subtitle School Form Specification.

Details

The Details card summarises the form:

Field	Meaning
Name	The form’s name
Country	The country it belongs to (a link)
Number of Assessments	Pupil assessments collected with this form
Number of Visits	School visits collected with this form
Number of Monitors	Monitors linked to it through their projects

Projects

The Projects card lists the projects that use this form, with their Number of Visits and Number of Assessments. There is also a Calendar card showing visits by date.

Monitoring Reports and Assessment Reports

The Monitoring Reports card lists the reports built from this form. On assessment forms only, an Assessment Reports card also appears.

What you can do here

Everyone can use these actions:

• Print (a dropdown): printable copies of the form. See Printing forms.
• Add to all country projects: a modal with a Name field and a Default toggle. It adds this form to every project in the country that does not already have it.

These actions appear only for administrators:

• Configure Form: opens the Form Configuration page, where you add and arrange questions and manage the teacher and pupil forms nested under this one.
• New Monitoring Report: create a report from this form (the name must be at least three characters).
• New Assessment Report: shown on assessment forms only, creates an assessment report.
