Configuring a form
The Form Configuration page is where the real work of building a form happens. Reach it from the Configure Form button on the School Form Specification view. The header shows the form’s name with the subtitle School Form Specification.
The actions below are administrator only (junior managers and above can also see the New Teacher Form button, though creating one still requires an administrator). The whole page is reached through an administrator-only button, so in practice only administrators get this far.
Header buttons
Section titled “Header buttons”- Edit: change the form’s Name (at least three characters).
- Delete Form: removes the form. A form that has been added to a project cannot be deleted; remove it from its projects first.
- Duplicate Form: opens a confirmation titled “Confirm duplicate school form spec” with a Duplicate button. It creates a full copy of the form, named after the original with ” Copy” added, including its questions and all of its teacher and pupil forms.
- Copy Form to Another Country: lists the organisation’s countries; choose one and confirm to copy the form into that country.
- Print Form: opens the printable school form. See Printing forms.
The Questions tabs
Section titled “The Questions tabs”Below the header buttons is a tab group with three tabs.
School Questions
Section titled “School Questions”The questions asked about the school. Each row shows the Question (a link), its Order, and edit and delete actions for administrators. A New Question button adds another.
Head Questions
Section titled “Head Questions”The same, for questions asked about the head teacher.
Teacher Forms
Section titled “Teacher Forms”A table of the teacher forms nested under this school form. Each row links to the Teacher Form and has a button group:
- Teacher Questions: the questions asked about the class teacher.
- Observation Questions: the lesson observation questions.
- Pupil Assessments: the pupil forms under that teacher form, each with its own questions and a New Pupil Form button.
Below the table, a New Teacher Form button creates another teacher form (field Name, at least three characters).
Adding a question to a form
Section titled “Adding a question to a form”Use the New Question button on any question tab. The New Question modal has:
- A Monitoring Question select. The choices are drawn from your question banks, filtered to the matching subject and excluding questions already on the form. They are grouped with separator lines that read like
--- [bank name] ---. If you do not pick one, you see the validation message “Please select a question”. - A sort-order field that sets where the question appears in the list.
Questions come from your banks, so if the one you need is not listed, add it to a bank first. See Question banks.
Adding a pupil form
Section titled “Adding a pupil form”From the Pupil Assessments area of a teacher form, use New Pupil Form. It has two fields:
- Name
- Assessment Definition: the test this pupil form runs, chosen from your assessment definitions.
